31 Jan INNOVATIVE PROGRAM FOR THE ELDERLY WITH ELDERCARE
AACMS has been asked by Eldercare to join them in delivering an enhanced service program to their nursing home residents.
The program aims to enhance the care of nursing home residents and prevent unnecessary hospital admissions by providing a more comprehensive and coordinated admission process to guide ongoing medical and care management, increasing on site catheterisation and hydration and finally enhanced palliative care.
