Comprehensive Health Assessments & GP Management Plans.
Where necessary our Doctors are able to conduct Comprehensive Health Assessments and GP Management Plans. These tools identify the specific needs of our patients, help evaluate the health status of individuals and assist in the management of chronic or complex conditions.
Medication Management
When required our patients regularly undergo ‘Residential Medication Management Reviews’ (RMMR’s). These reviews are conducted by an accredited pharmacist. RMMR’s are designed to provide a comprehensive assessment of a patient’s medication as well as identify, resolve and prevent medication-related problems. The report is then reviewed and actioned by the patients primary GP.
Bone density scans
Osteoporosis is a major clinical problem in older women and men, in Australia affecting over one million people. A ‘dual-energy x-ray absorptiometry’ (DEXA) scan is a widely used diagnostic tool that has the ability to measure bone density and assess the risk of bone fractures.
Upon GP request our administration team organise the Adelaide Bodyscan mobile DEXA van to visit our residential aged care facilities. For patients over the age of 70 the DEXA bone scan is funded by Medicare once every two years. Vaccinations
Our practice aims to ensure that all of our patients are provided with their up to date vaccines. Below are the three common vaccines that we provide to our patients;
- Influenza vaccine
- Pneumococcal vaccine
- Shingles vaccine
Hospital Admission Prevention
Our GP’s endeavour to provide responsive medical treatment with the aim at reducing hospital visits. Unnecessary hospital visits can often be highly distressing for patients and their families, as well as detrimental to the patients overall wellbeing. Two of our facilities take part in the initiative called ‘The Dandelion Project’ which is run by the Adelaide Primary Health Network (Adelaide PHN). This projects aim is to avoid hospital transfers for residents who could be better treated in the comfort of their facility. Our GP’s contribute to this initiative in the following ways;
- Preparing and reviewing patient advanced care plans.
- Responding to acute clinical events.
- Engaging in collaborative health care.

Palliative Care
We aim to provide a patient and family-centered approach during the end of life stage. Our GP’s endeavor to do the following during this time;
- Respecting the patient wishes, needs and dignity.
- Ensuring that the patient is as comfortable as possible.
- Providing support for emotional, social and spiritual concerns.
- Providing support for the family.
- If necessary referring patients to specialised palliative care services.
